Submission Instructions

Emails must be valid.

Your policy number can be found on your policy schedule. All fields are required in order to activate your account.

The form will not submit without the correct Validation Code.

On submission you will receive an activation email to complete the sign up process.

Validation code - required as this service is exclusively available to Accelerate policyholders on the Choices product. The validation code can be found on page 3 of your policy wording.
